Agreement to terms
These Terms of Service (Terms) are an agreement between you and HealthForge Inc. (HealthForge, we, us). They govern your use of our website, applications, and related services, which we call the Services. By creating an account or using the Services, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Services.
Eligibility
You must be at least 18 years old to use the Services. By using them, you confirm that you are 18 or older and able to enter into this agreement.
The Services
HealthForge brings your health data into one record, reads it with a personal AI, and can connect you with longevity clinicians. Some features, including clinical and telehealth care, are provided through licensed clinicians and may be offered as paid plans. We may add, change, or remove features over time.

The HealthForge app and its AI provide information and organization to help you understand your health. They do not provide medical advice, diagnosis, or treatment, and using the app does not create a doctor-patient relationship.
Clinical care, when you choose it, is provided by licensed clinicians through our telehealth services and is subject to their professional judgment and separate consents. Always talk with a qualified clinician before making medical decisions. If you think you may have a medical emergency, call 911 or your local emergency number.
Your account
You are responsible for the activity on your account and for keeping your login credentials secure. Tell us promptly if you suspect unauthorized use. We may suspend or close accounts that violate these Terms or that create risk for other users.
Subscriptions and billing
Paid plans, including telehealth plans, are billed in advance through our payment processor. The price that applies to you, including any recurring amount and billing period, is shown before you subscribe.
Paid plans renew automatically until you cancel. You can cancel anytime, and cancellation stops future charges. Fees already paid are not refundable except where required by law or stated otherwise. Prices may change, and we will give notice before a change affects you.
Acceptable use
Use the Services only for lawful purposes and as intended. Do not misuse the Services, interfere with their operation, attempt to access them in unauthorized ways, or use them to harm others. Do not upload content you do not have the right to share.
Your content and data
You keep ownership of the information and content you submit to HealthForge. By using the Services, you grant HealthForge a worldwide, perpetual, irrevocable, royalty-free, fully transferable, and sublicensable license to host, store, reproduce, modify, adapt, translate, create derivative works from, de-identify, aggregate, analyze, and otherwise use and exploit your information and content for any purpose permitted by law. This includes operating, developing, and improving our products and services, developing, training, and improving artificial intelligence and machine learning models, and research and commercial purposes.
HealthForge exclusively owns all de-identified and aggregated data, and all models, algorithms, insights, analyses, improvements, and other materials that we develop or derive from your information and content, together with all intellectual property rights in them.
Closing your account does not require us to delete your information from our systems. We may retain and continue to use your information, and the data and materials we derive from it, as described in our Privacy Policy. Where applicable law requires deletion of personal information, we will comply to the extent required and may retain de-identified and derived data. The licenses and ownership described in this section survive the closing of your account and the termination of these Terms.
Third-party services
The Services work with third parties you connect, such as wearables, apps, and labs, and rely on providers such as hosting and payments. We are not responsible for third-party services, and your use of them is subject to their own terms.
Intellectual property
The Services, including our software, design, and brand, are owned by HealthForge and protected by law. These Terms do not give you rights to our intellectual property, except the limited right to use the Services as intended.
Disclaimers
The Services are provided as is and as available, without warranties of any kind, whether express or implied, to the fullest extent allowed by law. We do not warrant that the Services will be uninterrupted, error free, or fit for a particular purpose. This does not limit the professional responsibilities of licensed clinicians who provide care.
Limitation of liability
To the fullest extent allowed by law, HealthForge will not be liable for indirect, incidental, special, or consequential damages, or for lost data or profits, arising from your use of the Services. Our total liability for any claim relating to the Services is limited to the amount you paid us for the Services in the twelve months before the claim.
Indemnification
You agree to indemnify HealthForge against claims and costs that arise from your misuse of the Services or your violation of these Terms or applicable law.
Dispute resolution
If you have a concern, contact us first and we will try to resolve it. If we cannot resolve it informally, any dispute will be handled under the governing law below and in the courts located there, unless you and HealthForge agree otherwise in writing.
Governing law
These Terms are governed by the laws of the State of Delaware, without regard to its conflict-of-laws rules.
Changes to these terms
We may update these Terms as the Services change. When we make material changes, we will update the date above and, where appropriate, notify you. If you continue to use the Services after a change takes effect, you accept the updated Terms.
